Licensing and registration

Have you lost or had your licence stolen? If so, ESV will be happy to replace your licence card.

In order to have your card replaced, we do require a statutory declaration from you.

Your local pharmacy, legal firms, police station will be able to assist you with a statutory declaration. Once this is completed, please forward to:

The declaration should clearly state your current address information as well as your licence number to be reissued.
